Healthy for Life
Health Care Access Now / Chronic Disease Care Coordination Pathway Collaborative - working to improve health care outcomes through integrated care coordination for adults diagnosed with diabetes, hypertension and/or high cholesterol. United Way is a partner in the effort led by Health Care Access Now.
In addition to a network of programs funded at local partner agencies, initiatives include:
Family Violence Prevention Project - a community collaborative of more than 40 public and private agencies, led by the YWCA of Greater Cincinnati.
Community Health Agenda - The Health Collaborative is leading an effort of key stakeholders in our region to develop a long-term, systematic plan to drive better health, better health care and lower costs for our community.
Cradle Cincinnati - A newly-formed partnership focusing on comprehensive, evidence-based strategies to reduce the infant mortality rate in Cincinnati and Hamilton County.
